- Ryan White Program recipient: Benefits include primary medical care and essential support services for people living with HIV who are low-income, uninsured, or underinsured.
- Care Management Program: Community Health Workers create connections between our vulnerable client population and their AHF Healthcare Center healthcare providers.
- Medical Care Coordination: Assist individuals and families living with HIV/AIDS in Los Angeles County. Improves health outcomes and prevents new HIV infections through Individual Care Plans that include their medical and mental health, as well as housing, transportation, public assistance, substance abuse support, and outside medical services.
